Identifying Quality Practices in Care Coordination for the Birth to 3 Program 

The Birth to 3 Program is an early intervention program for eligible infants and toddlers, ages birth to 3, who have developmental delays and disabilities. The program provides support to parents and caregivers, assisting them in meeting the unique needs of their children. MetaStar conducts Birth to 3 protocol reviews on behalf of the Wisconsin Department of Health Services to identify quality practices in care coordination provided to program recipients and their families. 

MetaStar’s experienced team identifies review criteria and evaluates information from program recipients’ records to identify quality practices. The software development team creates secure web-based tools to gather results or reviews, providing data and narrative reports to identify strengths and opportunities at agency and statewide levels.
